Quite possible that kitty can’t groom himself very well. Surprisingly 50%-90% of cats over age 4 have some kind of dental issue. One of my cats was allergic to his own teeth and had to get them pulled out one by one over time until now he has no teeth anymore, and due to his history of a hurting mouth, he learned not to clean himself because it hurt so much. Apparently that’s relatively common issue too. Wouldn’t be surprised if this guy is a little scruffy from dental pain.
